Community Services Coordinator
Position Summary
Rutgers, The State University of New Jersey, is seeking a Community Services Coordinator in the Department of Emergency Medicine within the New Jersey Medical School (NJMS).
The Community Services Coordinator for the Mobile Community Health (MCH) program provides direct community-based outreach and overdose response services while serving as the operational lead for the MCH team. This role combines community engagement, care coordination, and research implementation responsibilities.
Key Duties
- Conducts community outreach to connect individuals with opioid treatment programs and MOUD providers.
- Responds alongside EMS providers to 911 and overdose calls, providing care navigation, support services, and linkage to care.
- Provides education, guidance, and navigation support to individuals with opioid use disorder.
- Coordinates follow-up care and referrals to medical, behavioral health, and social service providers.
- Collaborates with hospital and community partners to ensure seamless transitions from the Emergency Department to community-based care.
Minimum Education and Experience
- Bachelor's Degree in Public Health, Human Services, Social Work, or a related field, plus three (3) years of relevant experience in coordination, patient navigation, or community-based services.
- Equivalent experience, education and/or training may be substituted for the education requirements.
