Medical Case Manager & Educator

About the Job

Health Education (35%):

Educational Group Development-

  • Create, implement, and evaluate educational group activities and curriculum (over video-conferencing when necessary)
  • Collaborate with external agencies to develop comprehensive curriculum across community agencies
  • Post at least weekly to social media accounts, Facebook, Instagram, Twitter
  • Coordinate the logistics of groups, including: venues, refreshments, and transportation
  • Provide additional individual HIV/STI and/or benefits education to clients as needed
  • Identify, vet, and recruit guest speakers for education groups

Program Recruitment-

  • Identify and cultivate key means to recruit potential clients (cold calling, flyers, social media posts), and work with care teams to recruit additional participants
  • Research and curate social media posts to engage with potential clients
  • Address transportation barriers for clients, utilizing knowledge of available community programs

Monitoring, Evaluation, and Scholarship/Research-

  • Evaluate program for quarterly reports, writing program narratives and analyzing data for submission to grant agencies

Medical Case Management (35%):
Client Management -
Using a positive youth development frame, case managers will empower clients by the following:

  • Evaluate potential clients for eligibility criteria, including: HIV status (or relation to an eligible PLWH), income, Ryan White eligibility, and age
  • Enter into a formal medical case management relationship with eligible clients and persons related to the client, if desired by client
  • Conduct initial assessment and ongoing re-assessment of clients’ psycho-social and economic needs
  • Navigate intersectional barriers to overall client well-being through: application assistance, referrals to external programs, motivational interviewing, conflict resolution/transformation, relationship building, and resource coordination/navigation
  • Conduct rapid HIV testing and linkage to care, as needed to assess potential clients’ eligibility for programs
  • Assist with programs and events as needed

Medical Care Coordination -

  • Conduct initial assessment and ongoing re-assessment of client’s medical needs and barriers to care
  • Manage clients’ medication and medical appointments- while coordinating with clients’ care team
  • Coordinate transportation to medical appointments, pharmacy, labs, and other medical services as needed
  • Enroll clients into the Treatment Adherence intervention when applicable

Monitoring, Evaluation, and Scholarship/Research-

  • Ensure clients are attending medical appointments and obtaining medical labs in accordance with physician recommendations
  • Monitor clients’ viral load, CD4 count, and appointment attendance
  • Guarantee client medical care meets state program requirements for appointment and bloodwork frequency

Treatment Adherence Counseling (10%):
Treatment Adherence Intervention -
In addition to the above responsibilities:

  • Evaluate current YAP clients for eligibility criteria, including: HIV status (with a viral load >200), and/or two or more consecutive missed medical appointments, and/or the client expresses a desire for additional assistance with treatment. All new clients new to YAP are enrolled without consideration for these factors
  • Conduct initial assessment and ongoing re-assessment of client’s barriers to treatment adherence and appointment attendance
  • Manage clients’ medications, and medical appointments- while coordinating with clients’ care team
  • Navigate barriers to treatment adherence and appointment attendance though: ensuring client medical literacy, care navigation and coordination, motivational interviewing, troubleshooting adherence issues, providing adherence tools, goal planning, and relationship building
  • Contact clients weekly or monthly, depending on progression through the intervention

Monitoring, Evaluation, and Scholarship/Research-

  • Monthly reporting to program leadership on client progress and enrollment

Administrative Tasks (for both above responsibilities) (10%):
Program Development and Management-

  • Take part in care conferences with YAP case managers, HIV medical providers, clinical pharmacists, and clinic social workers
  • Data tracking and entry into internal and external databases with a high level of accuracy
  • Participation in weekly staff consultations and monthly reporting
  • Assist with quarterly reporting to grantors and other funders

Community Engagement and Health Professions Learner Education (5%):

  • Collaborate with other health professions learners as applicable, for example social work interns or pediatric fellows
  • Opportunities for leadership positions in community committees, as desired and available

Professional Development and Continuing Education (5%):

  • Attend HIV trainings and conferences as required, recommended, and available
  • Maintain knowledge of (and attend training on): resources available to target populations, motivational interviewing, and treatment adherence strategies

Essential Employees

The successful candidate will be designated as an Essential Employee. Essential Employees are expected to report to campus during a University State of Emergency (reduced operations or campus closure) unless specifically directed by the Department not to report or if directed by civil emergency or medical authorities not to report for health and safety reasons. As an Essential Employee, you must report to campus even if the closure is less than 3 days.

Location: In-Person (Minneapolis, MN)

Qualifications

All required qualifications must be documented on application materials

Required Qualifications:

  • BA/BS degree or a combination of related education and work experience to equal at least four years.
  • Demonstrates the ability to work with individuals in social services, public health, human services, or a related setting that involves providing direct support, case coordination, or client-centered services.
  • Demonstrates the ability to work with or support people living with HIV or other populations with complex health or social needs.
  • Must pass criminal and driving record checks
  • Must use personal vehicle for client transportation and maintain comprehensive insurance

Preferred Qualifications:

  • Knowledge of HIV disease
  • Ability to work flexible hours including some weekends
  • Cultural responsiveness while working with multicultural, low-income individuals and families
  • Understand and adhere to all organizational policies regarding informed consent, privacy and confidentiality

Pay and Benefits

Pay Range: $46,280 - $56,160/year; depending on education/qualifications/experience

Time Appointment: 100% Appointment

Position Type: Civil-Service & Non-Faculty Labor Represented Staff
