Job Summary
The Nurse Navigator works closely with physicians in various specialty areas and coordinates a patient's plan of care, communication, and program. Position provides expert individualized care coordination to patients and their families ensuring a timely schedule for diagnostic work-up. Position also communicates between the patient and the multidisciplinary team ensuring the patient receives all the necessary referrals and follow-up.
Major Responsibilities/Duties/Critical Tasks
- Develop and enhance processes with the aim at improving the clinical experience for patients and their caregivers.
- Reviews referrals for appropriate testing and information prior to approval by Providers.
- Coordinate with information technology and quality departments to ensure quality measures are met and data is recorded accurately.
- Utilize clinical protocols to refer patients for appropriate screenings and diagnostic testing
- Maintain knowledge of current state, federal, and facility requirements to meet current healthcare goals related to patient
- safety, reimbursement, and healthcare quality standards.
- Work with a multidisciplinary team to develop and implement an up-to-date plan of care.
- Assess, develop, and implement effective patient and family teaching to accurately assess readiness to learn.
- Communicate with all members of the healthcare team on behalf of the patient, caregivers, outside entities that are included in the care of the patient
- Assist the patient with referrals to the patient financial counselor when needed
- Collaborate with the marketing and outreach departments to educate referring physicians and local communities about the facilities and services provided.
- Establish quality improvement projects with residents and fellows.
- Maintain a patient database and participate in patient recruitment for clinical research.
- Other duties as assigned.
Required Education/Experience
- Graduate from an accredited school of nursing.
- Nurse Navigator experience
Required Certifications/Licenses
- Certified RN. (Must possess and maintain a current license or valid permit to practice in TX)
- Possess and maintain current BLS Certification.

The retirement plan for this position is Teacher Retirement System of Texas (TRS), subject to the position being at least 20 hours per week and at least 135 days in length.
This position is security-sensitive and subject to Texas Education Code Section 51.215, which authorizes the employer to obtain criminal history record information. Applicants selected must be able to show proof of eligibility to work in the United States by time of hire.
