Position Summary

The Patient Navigator within our Sickel Cell unit provides education, guidance, and emotional support to patients and families as they navigate the healthcare system. This role serves as a primary point of contact for referring physicians, patients, and caregivers, helping connect them to hospital and community-based supportive services. The Patient Navigator works closely with care teams to promote care plan adherence, access to resources, and continuity of care.

Key Responsibilities

  • Meet with or contact patients and families to review care plans and provide education on hospital and community resources.
  • Assess patient needs and coordinate referrals to internal and community-based services.
  • Serve as a liaison between patients, providers, and support services to ensure access to appropriate care.
  • Assist with scheduling multidisciplinary appointments and coordinate logistics such as reminders and transportation.
  • Support patients with insurance applications and provide relevant eligibility information to care teams.
  • Maintain collaborative relationships with physicians, nurses, social workers, office staff, and community partners.
  • Document patient interactions, collect required data, track outcomes, and support reporting and planning initiatives.
  • Participate in care planning meetings to support discharge planning and ongoing care coordination.

Minimum Qualifications

  • Education:
    High School Diploma or GED required
    Bachelor's Degree preferred
    Preferred area of study: Human Services or related field
  • Experience:
    Minimum of one year of closely related experience required
    Experience working with clinic populations or community resources preferred
  • Knowledge, Skills, and Abilities:
    Strong written and verbal communication skills, with the ability to demonstrate empathy in challenging situations
    Analytical and problem-solving skills
    Ability to manage data documentation and reporting
    Spanish fluency preferred

Full-time, onsite

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors. Base pay is one part of a comprehensive benefits package that includes flexible schedules, affordable health, vision and dental insurance, child care and student loan subsidies, generous levels of time off, 403(b) Retirement Savings plan, Pension, Tuition and certain License and Certification Reimbursement, cell phone plan discounts and discounted rates on T-passes.
