Job Summary

This position will serve as a functional member of the Continuous Quality Improvement Team in implementation of program goals and objective. This role involves a systematic approach to monitoring, assessing and improving health care quality at SIU Center for Family Medicine-Carbondale. Primary functions will include review of charts/processes, gathering/collecting data, working reports, assisting with the assessment and monitoring of quality metrics, the teaching of preventive medicine, and teaching at outreach programs. This position will work closely with the Medical Director, Director of Quality, Charge Nurse, as well as the Health Care Administrator and will collaborate with physicians, residents and other members of the health care team and patients to promote the delivery of quality, efficient and cost-effective healthcare services in a patient-centered environment.

Clinical Services - 50%

  1. Obtains queried disease-specific and preventative maintenance reports and works lists, reviewing charts and contacting patients via phone and/or letter to notify patients in need of particular services. Assists patients with compliance of identified services and in overcoming barriers to gaps identified.
  2. Participates in clinic team activities, such as huddles or quality initiatives to communicate and share information with the health care team that will assist in both proactively addressing service needs and also assist in shoring up gaps in care.
  3. Collaborates with physicians to enhance evidence-based clinical guideline adherence and promote best practice by initiating and/or adjusting therapies and clinical interventions as directed by the physician and providing appropriate follow-up and monitoring as needed.
  4. Works with providers and support staff to assist in identifying and implementing reports or processes that improve follow-up of care, such as laboratory and diagnostic testing, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
  5. Assists in collecting/gathering data on other quality metrics that advances improvement processes such as determined by the Quality committee, relevant to UDS measures or aligning with other quality indicators.
  6. Assists with chart audits relevant to advancing population health and some subpopulations.
  7. Works in the Lincoln Scholar Program coordinating a multispecialty group of learners and students for quality care of patients, as well as coverage for other providers on the floor as necessary.

Administrative/Teaching Services- 50%

  1. Assists in creating, trending and generating quality reports. Reports on recalls, FDA bulletins, drug reports, drug interactions & safety reports.
  2. Attends and reports at Quality meetings, staff meetings and conferences as requested by the Director of Quality, Risk & Safety, PCMH, CQI, and Performance Improvement Team (PIT).
  3. Works directly with the faculty/residents/staff toward improvement of identified processes. Including but not limited to UDS metrics, Care Coordination, and preventive medicine.
  4. Other duties as assigned.

MINIMUM QUALIFICATIONS:

  1. Bachelor's degree in Nursing preferred, minimum RN, Health Care Administration, or a closely related field.
  2. One year of Quality Improvement work experience in a clinical or healthcare related field. Possession of an Illinois license as a registered nurse with the Illinois Department of Registration and Education.
  3. Ability to work as a member of a multidisciplinary team.
  4. Excellent customer service skills
  5. Problem solving abilities
  6. Strong written & verbal communications skills
  7. Possession of current CPR card.
  8. Sensitive to the needs of underrepresented minority populations.
  9. Able to work professionally and adeptly under pressure
