REGISTERED NURSE : Ambulatory Care, Post-Liver Transplant RN Coordinator
Responsibilities:
- Accurate and efficient documentation of nursing care
- Knowledge and appropriate use of internal and external resources.
- This RN position is essential in managing acute and chronic patient care for post-transplant patients. Provide skilled observations, telephone triage and health assessments of patients including history, appropriate interventions, and referrals.
- Foster continuity of care and facilitate transitions across outpatient and community providers on patient population.
- Potential to support outreach efforts.

- Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings.
- Proficiency with typing and electronic health record documentation.
- Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions.
- Willingness to adapt to different populations within Transplant Center after successful Orientation.
Required Qualifications:
- Current licensure to practice nursing in the State of Michigan.
- Three years of nursing experience as an RN within the last 5 years.
- Care of immunocompromised patients within the last 5 years.
- Experience in caring for transplant patients as a RN within the last 2 years.
- Transplant Certification (CCTC/CCTN) within three years of hire.
Preferred Qualifications:
- Experience with using the United Network Organ Sharing system.
- 2-5 years' experience in the ambulatory care setting.
- 1-3 years ICU experience
- Recent 3 years nursing experience in the inpatient setting
- Experience in the acute care of transplant patients
- Bachelor of Science Degree in Nursing.
Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework LEVEL C.
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level F.
Hours/Week: 40 hours
Shift/Hours/Days:
- Day Shift: Monday- Friday; potential weekends and holidays as clinic needs arise.
- Hours may be variable (based on patient and volume needs)
Location:
- Clinic Taubman Center Transplant Clinic
- On-site workspace located in UH-South
- Potential off-site outreach clinics
- Subject to change location based on patient needs
Note: All new employees will be expected to float to a designated unit(s) in times of low census.
