REGISTERED NURSE: Ambulatory Care Services (Intake Coordinator Adult Bone Marrow Transplant & Leukemia Clinic)

• Professional RN with clinical knowledge specific to the multidisciplinary care of hematology oncology patients, who provides individualized assistance to patients, families, and caregivers.
• Obtain accurate clinical and logistical information, allowing for safe triage and efficient first contact. (Referral review, pathology reports, radiology reports, and laboratory data, Verification of pathology/diagnosis, Risk Scan/Urgency Triage, Identify potential research opportunities / clinical trials)
• Support clinical triage and new referrals to facilitate appropriate scheduling, patient needs and timely access to care in collaboration with the scheduling intake team.
• Identify potential research opportunities and clinical trials.
• Perform Clinical Intake & Symptom Assessment including but not limited to, symptoms reported, severity of symptoms, functional status, comorbidities, current medications and allergies, and pregnancy and fertility considerations.
• Identify psychosocial status, perform financial screening and identify barriers to care such as health literacy challenges, transportation issues, financial/insurance issues, family/caregiver limitations, cultural/linguistic/spiritual needs, internet/phone stability, housing and food insecurity.
• Provide education and emotional support to patients and their families to help reduce the anxiety that a new diagnosis of cancer may bring prior to the new patient's appointment.
• Promote seamless and timely patient-centered care from the time of referral, through the new patient appointment.
• Provide guidance to promote positive outcomes and patient satisfaction.
• Provide culturally sensitive, age-specific team-based care models in delivering nursing care for the oncology patient population.
• Accurate and efficient documentation of nursing care.
• Knowledge and appropriate use of internal and external resources.
• Translate complex medical information into language patients, families, and caregivers can understand and apply.
• Participates in the tracking and monitoring of metrics and outcomes, in collaboration with clinical administration to document and evaluate program outcomes.
• Serve as an essential liaison for patients from referral to new patient appointments.
• Responsible for outreach efforts to establish and maintain positive working relationships with key members of the healthcare team.
• Cross coverage expectations with other intake positions as needed

• Willingness to adapt to different populations within the team as patients? demands require. Orientation and training would be provided for new subspecialties.

Requirements of the Commission on Cancer Certification must be met within the triannual certification window proportionate to hire or transfer to the oncology area, details provided by leadership.

• Current licensure as a Registered Nurse in the State of Michigan.
• Minimum of five (5) years of Oncology RN experience within the last six (6) years
• Minimum of five (5) years Ambulatory Care experience within the last six (6) years

• Experience with patients undergoing lymphoma and myeloma treatment in the last 5 years.
• Experience with patients undergoing BMT/Leukemia treatment in the last 5 years.
• Experience as a Nurse Navigator or Clinical Care Coordinator in the last 5 years.
• Ambulatory Care Experience with telephone triage/symptom management within the last five (5) years.
• BSN
• OCN certification

Salary & Nursing Framework Level:
This UMPNC RN posting is posted as Nursing Framework LEVEL C.
Actual Nursing Framework LEVEL and salary will be determined at time of hire.
Nursing Framework levels range from Level A to Level F.

Hours/Week: 40 hours/ hours based on clinic needs

Day Shift; Monday-Friday, may include Saturdays/Sundays/Holidays in the future if clinic hours expand
Hours may be variable (based on patient and volume needs)
