REGISTERED NURSE : RN Coordinator- Decentralized Float- Adult Transplant
- Utilize the nursing process to facilitate patient care for disease management as well as through pre-transplantation stages in clinic, procedure, and office settings as needed.
- Provide skilled observations, telephone triage, and health assessments of patients, including history, appropriate interventions, and referrals.
- Foster continuity of care and facilitate transitions across outpatient and community patient records for the patient population.
- Provide consultation and participate in problem-solving efforts with nursing staff, medical staff, referring physicians, and other disciplines.
- May be involved in the care or coordination of pre-transplant biopsy, paracentesis, and infusion patients.
- Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, the Joint Commission, and UNOS to ensure regulatory compliance in nursing practice.
- Foster continuity of care and facilitate transitions across outpatient and community settings.
- Utilize various resources available within the Ambulatory Care and Department of Internal Medicine/hepatology transplant settings to determine and meet the needs of the patient/family.
- Develop collaborative mechanisms with appropriate others (nurse supervisors, peers, etc.) for providing patient care.
- Includes transplant outreach clinic duties, which will require travel to outreach site locations. This may include outreach locations in Ohio.
Expectations:
- This position will be responsible for case coordination through patient evaluation, assessment, and implementation of planned interventions in clinics, procedures, and office settings.
- This position may facilitate patient education through telephone intervention and face-to-face meetings in both individual and group settings, and work collaboratively with the healthcare team to provide quality care in the outpatient setting for the transplant patient populations.
- Provide care to adult patients in a variety of settings at intervals that meet the needs of the transplant programs within the transplant center.
- Utilize various resources available within the Ambulatory Care and Department of Internal Medicine/nephrology transplant settings to determine and meet the needs of the patient/family.
- Monitor nursing care provided and utilize results of quality monitoring to initiate changes in practices, as indicated.
- Assume responsibility for ongoing professional development, attending appropriate continuing education, staff development programs, conferences, and grand rounds as indicated.
SUPERVISION RECEIVED
This position is under the direction of the Transplant Ambulatory Care Senior Nursing Director and Nursing Supervisors and will provide Ambulatory Care Nursing support to a variety of organ groups or procedure areas within the Transplant Ambulatory Care Clinic/Center.
Required Qualifications
- Current licensure to practice nursing in the State of Michigan.
- 3 years of nursing experience as an RN in the last 5 years.
- 1 year of ambulatory care experience within the last 3 years.
- Experience in caring for transplant patients as an RN within the last 2 years
- Transplant Certification (CCTC/CCTN) within three years of hire.
- Must be willing to obtain Ohio RN licensure.
- Ability to travel off-site to Outreach Clinic locations.

Preferred Qualifications
- 1-2 years experience caring for abdominal transplant patients within the last 5 years.
- 1-2 years of nursing experience in Ambulatory Care within the last 3 years.
- Ability to interact collaboratively with all members of the interdisciplinary team effectively and manage multiple tasks as demonstrated by interviews, evaluations, and references.
- Bachelor of Science Degree in Nursing.
Salary & Nursing Framework Level: This UMPNC RN posting is posted as Nursing Framework LEVEL C. Actual Nursing Framework LEVEL and salary will be determined at time of hire. Nursing Framework levels range from Level A to Level F.
Hours/Week: 40 hours/week Shift: Day shift, Monday-Friday; potential weekends and holidays as clinic needs arise. Hours may be variable (based on patient and volume needs)
Location: On-site work located in the Transplant Center. Work location could change based on patient needs, which includes coverage at outreach clinics in Michigan and Ohio.
Note: All new employees will be expected to float to a designated unit(s) in times of low census.
