Your Role on the Team
Position Summary:
Under the direct supervision of the Clinical Coordinator of Wound Care Department, provides patient with care that is continuously evaluated, planned for, and revised as needed; patient's status is documented within the reassessments; patient care needs; interventions to meet the patient's needs; nursing care provided; the patient's response to; and the outcome of care provided; organizes and prioritizes work to provide health information, discharge planning, and patient care in a safe environment to maximize health in the continuum of care and protect the patient; assumes responsibility for and assigned group of patients and be accountable for an assessment of the patient's basic human needs, enabling team of providers to make appropriate decisions to develop, implement and evaluate a plan for care, to include biophysical needs, bio-psychological needs, environmental/safety needs, self-care needs, educational needs, discharge planning needs; professional and competent as a care provider, a leader of personnel, and a collaborator with other professionals; demonstrates awareness of his/her unique position in the Department and responsibilities as a professional nurse and R.N. Case Manager through accurate, clear, concise and timely written and oral communications.
What It Takes to be Successful
Required Qualifications:
- Ability to work every other weekend
- Current California RN license
- Must possess CWON, CWOCN or WOCN certifications
- All External Candidates must have a Bachelor of Science in Nursing
- Current American Heart Association BLS certification required
- Minimum 1 year experience as a wound nurse, with primary responsibility for wound assessment and treatment
- Must demonstrate customer service skills appropriate to the job
- Excellent written and verbal communication skills in English
- Ability to maintain a work pace appropriate to the workload
- Ability to establish and maintain effective working relationships across the Health System
- Working knowledge of Joint Commission and other regulatory requirements present in an acute care setting
- Ability to maintain flexibility and work well in a constantly changing environment
- Strong customer service orientation as well as written and oral communication skills
- Complete understanding of acuity levels for specific patient populations.
- Basic computer literacy and proficiency with MS Word and MS Outlook
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Preferred Qualifications:
- Knowledge of Wound Related products and procedures
- Minimum one (1) year hospital Medical/Surgical or Rehabilitation experience
- Knowledge of comprehensive wound management including wound assessment, formulation of treatment plan, recommendation of appropriate wound dressing, documentation, evaluation, education, data collection, surveillance and outcome data
- Current state license to practice as an RN
- Must maintain current provider CPR certification throughout employment
- ACLS certification preferred within one (1) year of employment or as available courses allow
- Wound care certification preferred
Physical Requirements:
- Work in an office environment with adequate light, heat and ventilation
- Able to complete a two-person lift; wheelchair to exam table
- Near visual and hearing acuity with or without accommodation to distinguish changes in patient physical status and/or read documents and computer screen
- May be required to work around caustic chemicals
