In this role, you’ll serve as a trusted resource - helping individuals and families navigate available services and make informed decisions that support independence and well-being. You’ll respond to calls and inquiries, providing clear, impartial information about community resources and programs. Using established screening tools, you’ll assess eligibility and authorize services
such as home-delivered meals, homemaker support, and caregiver respite. You’ll guide individuals through program orientation, connect them with appropriate services, and maintain accurate records of all interactions in a statewide database. Your work will also include maintaining up-to-date resource information, advocating for clients, and supporting outreach efforts to educate the community about available services and protections.
