Social Worker II- Emergency Department
Position Summary
At Stony Brook Medicine, our Social Worker are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement.
Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.
Duties of a Social Worker II may include the following but are not limited to:
- Psychosocial assessment of Social Work High Risk Criteria Cases.
- Effectuate discharge planning.
- Coordinate with Medical Teams, Community Partners and Transitional Care teams.
- Charting and documentation including training, data management, statistics.
- Assist patient financial services, including Medicaid and other entitlement programs.
- Serve as liaison between community agencies and hospital.
- Preparation of material as needed for guardianship, oversight agencies, surrogate decision making.
- Prepare weekly updates for Long Stay Committee.
- Lead therapeutic groups when appropriate.
- Supervision of professional social work students when appropriate.
- Actively involved in performance improvement activities including program development, data collection and analysis.
- All other duties as assigned.
Qualifications:
Required Qualifications:
MSW from an accredited Social Work Education School. NYS LMSW or LCSW. Three year's post Master's Social Work experience in Medical Health Care. Demonstrates skills in crisis intervention. Demonstrates understanding of ED discharge processes. Demonstrates excellent written and oral communication skills. Ability to work and coordinate with established Medical Treatment Team. Demonstrates understanding of discharge planning and Long Length of stay barriers and systems. Demonstrates good assessment and psycho-social skills. Working knowledge of Long Island and Metropolitan NY community resources. Demonstrates patient first, patient advocacy and service excellence. Computer literate. Ability to implement and run medical team rounds. Demonstrates understanding of community systems. Demonstrates understanding of project management and action planning. Demonstrates team work and team leader focus. Demonstrates understanding of data collection and statistics. Demonstrates ability to manage large and complex case loads. Understands role of Social Work and Case Management as it pertains to throughput and Length of Stay.
Preferred Qualifications:
Experience in Emergency Department Social Work. Multiple Medical Specialties in Social Work Health Care. Experience in Hospital, In-Patient setting, or skilled Nursing Facility. Discharge Planning experience. Diagnosis specific Social Work. Bi-lingual.
