Transplant Social Work position with primary assignment to the Adult Lung Transplant Program

Social workers are critical and essential to all phases of transplant care. This position is part of multiple dynamic medical and social work teams to assess, counsel, and provide education and psychosocial interventions to transplant patients and their support networks through all phases of transplant care. This position will have primary assignment to the Adult Lung Transplant Program, which includes inpatient and outpatient responsibilities, and will also be trained to provide support in all adult solid organ areas.

You will be responsible for:
- Comprehensive psychosocial assessments to assess transplant suitability and development of treatment plans to address barriers to care both pre- and post-transplant. This will include assessment and intervention of mental health and coping, substance use disorders, financial/insurance/medication needs, medical decision making, comprehension of medical recommendations, social support and caregiving needs, among other psychosocial aspects of care.
- Proactive case management of assigned caseload.
- Working with patients around issues of grief/loss, end-of-life decision making, treatment planning/goals of care, and adherence.
- Provision of referrals and follow up to community resources as needed.
- Collaboration with interdisciplinary team members in inpatient and outpatient settings for care coordination and programmatic initiatives.
- On-call availability during clinic hours to provide immediate and/or urgent psychosocial intervention with Transplant Center patients and families.
- Referral to and/or care coordination with appropriate resources for continuity of care and to address psychosocial needs.
- Participation in interdisciplinary meetings and rounds and presentation of psychosocial recommendations.
- Participation in transplant continuous quality improvement initiatives.
- Participation in team and department meetings.
- Development, evaluation, and collaboration on supportive programming to enhance the patient/family experience.
- Cross coverage support within Transplant Social Work team.
Required Qualifications:
- A Master's degree in Social Work from a school accredited by the Council of Social Work Education.
- A current license to practice social work in the State of Michigan.
- Medical social work experience.
Desired Qualifications:
- Previous Transplant Social Work experience.
- Significant medical social work experience.
- Significant experience with mental health and substance use disorders as well as knowledge of MH and SUD treatment resources.
- Experience with pediatric to adult transition.
- Skilled in developing rapport and effectively communicating with a diverse patient population.
- Experience advocating on behalf of patients and addressing barriers to care.
- Efficient, accurate assessment of patient needs and effective case dispositions.
- Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic.
- Independent, sound decision-making is necessary for success in this position.
- Effective collaboration with a large multi-disciplinary team.
- Proficiency in keeping accurate and timely clinical records.
This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00.
This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification
This position is covered under the collective bargaining agreement between the U-M and the United Michigan Medicine Allied Professionals (UMMAP), which contains and settles all matters with respect to wages, benefits, hours, and other terms and conditions of employment.
