Scope of Position
In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel. Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to practice area. Practices in multiple different practice settings including inpatient and outpatient hematology and oncology. Position is not dedicated to a specific team, but provides float coverage to multiple areas. Position serves to provide coverage for vacancies and employee leave time.
Position Summary
This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff). Must demonstrate competency in multiple subspecialties of hematology/oncology in both the inpatient and outpatient setting. Must be willing to float to more than one area on a daily, weekly, or monthly basis depending upon the needs of the department.
Minimum Qualifications
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred; or score a minimum of 40 points on Pharmacy's Competency Based Advancement worksheet.
