Job Summary:
This is an opportunity for an Advanced Practice Provider (APP) to see patients with kidney disease in an outpatient setting.
Key Responsibilities:
- Conduct comprehensive patient assessments, including medical history, physical examinations, and interpretation of diagnostic tests.
- Develop and implement treatment plan, monitor patient progress, and adjust interventions as necessary.
- Collaborate with a multidisciplinary team of healthcare professionals to provide patient-centered care.
- Educate patient and their families about kidney health, disease management, and treatment options.
- Participate in ongoing clinical research, quality improvement initiatives, and staff development activities.
- Maintain accurate and up-to-date electronic medical records.
Work Location:
Onsite - we are hiring two people; the first role is expected to work onsite and is located in Aurora, Colorado, and the second role is expected to work onsite in Longmont, Colorado.

Qualifications:
Minimum Qualifications:
- Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in Colorado
Preferred Qualifications:
- 2-3 years of relevant experience
- Outpatient experience in Nephrology
Knowledge, Skills and Abilities:
- Knowledge base in renal pathophysiology required to provide care in patients with chronic kidney disease
- Proficient in managing patients with kidney disease and related co-morbidities
- Strong clinical knowledge and experience in nephrology or related specialty
- Excellent communication skills and ability to establish rapport with patients and colleagues.
- Proficiency in interpreting diagnostics tests and formulating differential diagnoses.
- Commitment to evidence-based practice and continuous learning.
Anticipated Pay Range:
HIRING RANGE: $105,185 - $145,000, depending on years of experience as an APP or Nurse Practitioner
