Position Summary

The Clinical Care Coordinator ? NP will be responsible for working in the dynamic and innovative academic practice at NYP/Columbia University Medical Center. Rapidly growing department offers collaborative practice opportunities to establish an ambulatory care practice and participate in the development of new clinical services in the Department of Orthopedic Surgery.

Responsibilities

  • Work independently or collaboratively with the Attendings, Fellows, and Resident staff as a well as the nurses, therapists, social worker?s and other support personnel who take part in caring for orthopedic patients.
  • Care coordination for patients needing non-operative treatment
  • Assist in managing pre-operative care across other practitioners to optimize patients for surgery
  • Obtain relevant consultations from other services and follow up on the information.
  • Peer-to-peer consults to obtain surgical pre-authorization for testing or surgeries for all spine surgical patients
  • Rounds independently and/or with attendings on all orthopedic inpatients.
  • Chart preparation according to protocol and orders appropriate x-rays for office hours using the Electronic Medical Record.
  • Complies with Meaningful Use Measures.
  • Evaluates and diagnoses patients
  • Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.
  • Administers injections/Prescribes appropriate pharmacological and non-pharmacological treatment modalities.
  • Monitors effectiveness of prescribed intervention.
  • Remove/apply dressings, sutures, staples, slings and/or braces
  • See post-op patients for wound checks and post-op care and notify attending of findings if abnormal
  • Reviews post-operative discharge instructions with patients including regarding wound care and pain management.
  • Returns patient phone messages at designated times throughout the day; documents action taken in the Electronic Medical Record (EPIC).
  • Triage incoming telephone calls; instructs patient and family regarding treatment instruction, then documents action taken in the Electronic Medical Record (EPIC).
  • Oversees appointment bookings and ensures emergency situations are scheduled appropriately
  • Sends patient prescription requests to pharmacy and documents action in the Electronic Medical Record (EPIC).
  • Manage and oversee all spine attending in-basket tasks in EPIC
  • Fill out necessary forms, i.e. disability forms, workman's comp, handicap parking as needed; Assist with patient correspondence as needed, i.e. letters of medical necessity, school notes, insurance inquiries, etc.
  • Assist patients in meeting health needs by identifying potential or existing problems, exploring available alternatives and providing support to the patient and family members to make an informed decision regarding health care.
  • Facilitates the patient's transition within and between healthcare settings, e.g. admitting, transferring, discharging and home care including visiting nurse referrals.
  • Formulate patient care protocols and critical pathways.
  • Assist with development; participate in design and implementation of clinical research trials.
  • Participate in accordance with hospital policies.

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience plus five years of experience
  • New York RN licensure, New York Adult Nurse Practitioner (ANP), or New York Nurse Practitioner (NP) licensure and/or New Jersey Nurse Practitioner (NP) licensure
  • National Certification & DEA required

Preferred Qualifications

  • MS in Nursing preferred; other master's degrees considered.
  • Clinical doctorate
  • Prior experience in management of adults with orthopedic diseases in an Orthopedic Hip & Knee practice, physical medicine and rehabilitation practice or pain management practice
  • CPR/AED certification
  • Bilingual (Spanish/English)

Other Requirements

  • Ability to perform comprehensive history and physical examination
  • Ability to manage and coordinate a patient caseload
  • Work within a collaborative agreement/protocol with a physician and health care team
  • Demonstrate leadership ability to manage patients and interact effectively with peers and physicians
