Clinical Pharmacist Specialist - Ambulatory Care Pediatric Cardiology
Job Summary
To work collaboratively as part of a multidisciplinary team to develop, implement, and monitor clinical pharmacy services within the pediatric cardiology ambulatory clinics affiliated with University of Michigan Health. Responsibilities include providing ambulatory clinical pharmacy services such as disease management, comprehensive medication reviews, and transitions of care planning. The position will also support the development of clinical guidelines and policies, supporting staff training activities, leading quality improvement initiatives, and assisting in the clinical education and training of residents and students.
The individual must demonstrate a professional and positive presence when interacting with patients, faculty, and staff, while maintaining a high standard of personal and professional conduct. Strong interpersonal, communication, and problem-solving skills are essential, along with the ability to effectively engage with diverse patient populations and work collaboratively within a multidisciplinary team while managing multiple responsibilities in a fast-paced ambulatory care environment.
Responsibilities
- Provide pharmaceutical care by: participating in patient care with the medical staff during outpatient clinic visits; designing, implementing and monitoring patient-specific pharmacotherapy; promoting safe, evidence-based, and cost-effective use of drug therapy; developing and fostering positive collaboration with providers and clinic staff; documenting pharmaceutical care activities and outcomes.
- Assist in the development and implementation of quality improvement processes to monitor the effectiveness and quality of clinical pharmacists' services.
- Participate in the training of General Pharmacy Practice Residents, Ambulatory Care Specialty Residents, Pediatric Specialty Residents, and Pharmacy Students.
- Conduct or participate in drug use projects, cost reduction/management initiatives, and quality improvement activities consistent with the Health System’s needs and objectives.
- Individual or collaborative scholarly/research activity is encouraged.
Supervision
Supervision Received: Supervision is received from the Director of Ambulatory Clinical Pharmacy Practices.
Supervision Exercised: Functional supervision is exercised over Pharmacy Residents and Students.
Required Qualifications
- Doctor of Pharmacy Degree from an accredited College of Pharmacy or equivalent combination of education and experience
- Certificate of completion from a Specialty Residency in Pediatrics, fellowship, or an equivalent combination of education, training and experience in Pediatrics
- Current licensure as a registered pharmacist by the State of Michigan Board of Pharmacy
- Michigan Controlled Substance License to practice pharmacy in the State of Michigan, or eligibility to become licensed within 60 days of employment start date
- Demonstrated ability to collaborate on clinical care ideas across multiple patient care services and to communicate effectively as a patient advocate.
- Demonstrates a commitment to carrying out professional responsibilities, adherence to ethical principles, and sensitivity to diversity with a commitment to service excellence and Patient Family Centered Care.
Preferred Qualifications
- Board Certification in Pediatric Pharmacy (BCPPS)
- Ability to perform a variety of manual dexterity movements, including keyboarding and IV preparation, actively participating in codes.
- Fluency in reading, writing and speaking English.
Hours
40 hours, full-time, day shift
