Essential Job Duties

Key Responsibilities

• Attend and participate in trainings and meetings.
• Answer incoming calls and conduct medication adherence
• Make outbound telephonic calls
• Confirm patient appointments 24-48 hours in advance
• Collect patient demographics and pertinent information related to their health
• Relay clinical questions out of scope to supervising pharmacist
• Perform medication adherence services such as reviewing patient’s medications and whether their medications are being taken appropriately and on time
• Fill out Medicare documents (Cover Letter, Medication Action Plan, and Personal Medication List)
• Analyze individual metrics related to your performance
• Document findings and interventions
• Communicate with providers
