Job Summary
The South Main Clinic at the University of Utah School of Medicine has an immediate opening for a Social Worker that shares our vision and passion for caring for children and their families. The Social Worker will be responsible for integrating counseling services with overall patient care.
This position is located at the Department of Pediatrics South Main Clinic on 3690 South Main Street, Salt Lake City, Utah.

Responsibilities
Essential Functions:
- Conduct team-based well-child visits with the primary care provider; monitoring development, social-emotional functioning, and relational health, and providing anticipatory guidance.
- Work closely with pediatric primary providers around care coordination, goal setting, coaching, and education about key aspects of a child's development.
- Conduct consultations on a short-term basis for needs regarding development and/or behavioral concerns such as sleep, positive parenting, picky eating, etc.
- Maintain a family support line and responds to and tracks call requests within the designated response time.
- Participate in reflective supervision meetings.
- Conduct home visits and may accompany families to key medical, specialty, and community agency appointments as needed.
- Facilitate parent/caregiver groups.
- Maintain community resource directory/database.
- Provide referrals and track follow-up.
- Participate in community-wide early childhood and/or mental health meetings.
- Provide consultation and/or facilitate training to medical professionals and practice staff re: early childhood development, early relational health/infant early childhood mental health, and trauma-informed practice.
Minimum Qualifications
Required: Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire. One of the following: Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah. Current license to practice as a Certified Social Worker in the State of Utah. This position is patient-sensitive and must fulfill all associated requirements.
Preferences
Department Specific Preferences: Fluency in Spanish, both oral and written. Experience working with infants and toddlers (ages 0-3).
