Primary Duties and Responsibilities:
Medication Reconciliation Pharmacy Technician Duties:
- Conduct in-person or phone meetings with patients to update medication lists prior to their provider appointments, ensuring accuracy and completeness of the medication list.
- Practices as a pharmacy technician in accordance with all federal, state, DFCI and Joint Commission policies, regulations and standards.
- Create and update lists of current medications, document changes in dose, frequency, or route, and flag medications for removal to prevent harmful drug interactions.
- Communicate drug-drug interactions or other medication changes with pharmacists and providers to maintain patient safety.
- Manage medication lists for medications listed from external entities, ensuring comprehensive and accurate records.
- Utilizes multiple systems such as EHR (Electronic Health Record, Inventory Management Systems and other systems to complete and document duties. Troubleshoot as necessary.
- Trains other employees as assigned.
- Communicates and escalates issues or problems.
- May be asked to provide cross coverage in the hospital outpatient/specialty pharmacy as needed to maintain seamless patient care.
- Performs other duties as assigned.

- Ability to independently troubleshoot issues and assist in resolution or escalation to the appropriate resource.
- Participate in department meetings and functions to promote a successful and efficient pharmacy workgroup.
- Demonstrated critical thinking skills (ability to analyze information objectively and make a reasoned judgment).
- Strong attention to detail.
Minimum Job Qualifications:
Minimum Education: High School graduate or equivalent.
Minimum Experience: Two years of prior work as a pharmacy technician in a retail or hospital environment required. Experience with oncology medications preferred.
License/Certification/Registration Required:
Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required. National Pharmacy Technician Certification (PTCB) must be obtained within 1 years of hire date.
