Job Summary:
The Licensed Clinical Social Worker (LCSW) will be responsible for providing brief, evidence-based psychotherapies and supportive counseling to a select primary care clinic population. Treatment will be provided in-person at the CU Medicine Family Medicine - Centerfield clinic.
The Licensed Clinical Social Worker will provide short-term psychotherapy across the continuum of care, including identifying eligible patients based on defined criteria, providing psychosocial assessment, developing and executing treatment in alliance with patient's goals, and ultimately ending care with a firm discharge plan in place.
In addition, this role will be responsible for providing crisis support to clinic patients identified as needing urgent behavioral health needs and ensuring they are provided with and/or connected to the next appropriate level of care. In addition to providing brief psychotherapeutic interventions, this role incorporates social work-informed care management activities, including assessment of psychosocial needs, addressing social determinants of health, coordinating referrals, and supporting patient navigation across healthcare and community-based services to reduce barriers to care.
The Licensed Clinical Social Worker will work in collaboration with the behavioral health team as well as the primary care clinic's multidisciplinary team including the patient's primary care provider and clinic leadership.
Examples of Work Performed
The duties and responsibilities of the position include, but are not limited to:
- Work collaboratively with practice stakeholders to deliver integrated mental health services in a primary care setting.
- Provide short-term mental health treatment to targeted groups of patients. This includes developing comprehensive initial assessments, evaluating and measuring treatment goals, providing psychotherapy, and arriving at a mutually agreed upon discharge plan with the patient.
- Refer patients to higher level of care (e.g. specialty treatment) -or community long term care - as appropriate.
- Provide evaluation, appropriate crisis intervention and follow-up to patients within the primary care clinic who have been identified as at risk of harming themselves, another individual or are gravely disabled.
- Coordinate and revise treatment plan with input from interdisciplinary team and primary care provider.
- Provide routine patient progress monitoring.
- Support the management of patient referrals, including coordinating care for those patients not meeting criteria for clinic based mental health services.
- Support clinic in developing community and campus referral sources as needed.
- Work collaboratively with clinic staff to ensure efficient scheduling and placement of referrals.
- Provide social work-informed care management services, including assessment of psychosocial needs and social determinants of health; assisting patients with navigation of healthcare systems and community resources; coordinating referrals for social, financial, housing, insurance, and behavioral health supports.
- Other responsibilities, appropriate to this position, may be deemed necessary.
Work Location: Onsite - this role is expected to work onsite and is located in Aurora, Colorado.
Qualifications:
Minimum Qualifications:
- Master's degree in field relevant to licensure
- Current Colorado License as an LCSW.
- Three (3) years of professional clinical experience with adult populations.
Preferred Qualifications:
- Four or more years of experience independently delivering mental health care in a clinical environment for adults
- Experience working as part of an interdisciplinary care team.
- Experience working with diverse populations including, but not limited to, complex medical conditions and social/economic issues, Medicaid and Medicare populations.
- Experience with assessment, safety planning and placing M-1 holds for patient concerns related to harm to self, others, grave disability.
- Experience with assessment and follow-through of patient concerns related to mandatory reporting requirements.
- Experience working with an electronic medical record system (ex. Epic)
Knowledge, Skills, and Abilities:
- Knowledge of HIPAA rules and regulations, including limits of confidentiality.
- Clinical knowledge and skills that demonstrate understanding of DSM V and allow for appropriate diagnosis and assessment.
- Clinical skills which are well-rounded and encompass a variety of evidence-based modalities and theoretical orientations.
- Knowledge of assessment, safety planning and appropriate follow-up with patients who pose imminent risk to self and/or others.
- Some familiarity with psychiatric medications.
- Must possess excellent written and verbal communication skills and be proficient in writing mental health evaluations, progress and discharge notes.
- Excellent interpersonal and organizational skills with the ability to prioritize, multitask, and follow-up.
- Skilled with word processing and developing spreadsheets using Microsoft Office such as Word, Access and Excel.
