Patient Navigator: Enhanced Care Planning Research Study
VCU Department of Family Medicine and Population Health is seeking a part-time, non-benefitted, hourly employee for a Patient Navigator position with our Enhanced Care Planning Research Study. Patient Navigators will be joining a multidisciplinary research team that is testing enhanced care planning and clinical-community linkages in order to comprehensively address the basic needs of patients with multiple chronic conditions.
Duties will include:
- Help patients create a care plan to address unhealthy behaviors, mental health, and social risks that might be impacting their health.
- Call patients once a week to support progress on their care plan and help them meet their goals.
- Connect patients to a community health worker for community resources to better help patients achieve their goals.
- Additional responsibilities as assigned.
Previous or current experience with only one of the following is required:
- Clinical research experience
- Experience as a research nurse
- Experience as a Physician’s assistant
- Medical student, intern, or resident
- Graduate degree in psychology
Anticipated Hourly Rate: $20.00 - $25.00 per hour
This position is not open to full-time VCU employees. Regardless of the funding source, total hours worked by wage employees at VCU are limited to 1,480 hours or less over the Standard Measurement Period (May 1 through April 1st of the following year,) including regular and overtime hours, and any other hours worked in an overload or secondary job.
