Peer Case Manager
Under the supervision of the Program Director, in partnership with the San Francisco Department of Homelessness and Supportive Housing (HSH), the candidate will provide mentoring and support to individuals who have been recently housed in scattered site housing through the Flexible Housing Subsidy Pool Program. Clients may be struggling with one or several of the following: mental illness, substance use disorder, poverty, legal issues, discrimination, and experience in the homeless response system. The Candidate works with clients individually and in group settings with a focus on peer counseling, housing navigation and support to maintain housing. Candidate applies personal experience as a consumer of mental health services and the homeless response system. Assists coworkers with case management responsibilities; serves as a role model of recovery; utilizes a team approach to accomplish goals. Adheres to all State, Federal and County documentation standards and HIPAA regulations. Certification or credentialing in peer support services, substance abuse treatment and/or mental health services is strongly preferred. Works in collaboration with UCSF, core partners Brilliant Corners, and housing providers. Candidate demonstrates a commitment to anti-racism and social justice and the ability to engage and support diverse communities.
The salary range for this position is $29.99 - $48.23 (Hourly Rate).
