The Ambulatory Care Clinical Specialist in Infectious Diseases (ID) is an Advanced Pharmacist Practitioner who works with a multidisciplinary team to provide comprehensive medication management for ID patients. Responsibilities include medication reconciliation, evaluation of medications for appropriateness, side effects management, patient education on specialty medications and infusions, longitudinal follow-up and therapy outcomes monitoring, and assistance with patient access.

The Pharmacy Specialist in ID achieves positive patient outcomes through disease management guided by established protocols, promotes adherence, and enhances satisfaction among both patients and providers as evidenced by outcome evaluations. Following updated clinical guidelines, such as those from the Infectious Diseases Society of America (IDSA) and Cedars-Sinai’s P&T-approved protocols, the specialist initiates, adjusts, or discontinues therapies, orders and interprets lab results, and authorizes medication refills.

Additionally, this specialist educates patients about specialty medications and serves as a clinical resource for physicians and support staff. They also help develop and maintain clinical pathways, best practices, protocols, competency assessments, and support ongoing quality improvement efforts.

Qualifications

Educational Requirements:

  • Graduate Degree - Graduate of an accredited School of Pharmacy - Minimum
  • Completion of a PGY1 ASHP accredited residency program (or equivalent work experience)
  • Completion of a PGY2 ASHP accredited residency program in ambulatory care or infectious diseases - Preferred

Licenses and Certifications:

  • California Pharmacist license - Required
  • Advanced Pharmacist Practitioner (APP) License - Required or must be eligible for and obtain within twelve (12) months of hire
  • Board certification in relevant practice area (e.g., BCIDP, BCACP, BCPS) - Required or must be eligible for and obtain within eighteen (18) months of hire
  • Professional certificate in relevant practice area(s) - Preferred

What’s Else Are We Looking For?

  • Clinical Practice Experience: Previous experience in ID specialty or infusion therapy management in an ambulatory care setting is preferred.
  • Patient Experience: Possesses excellent patient service skills prioritizing patient care needs to support safe effective medication use.
  • Team Skills: Collaborates effectively with internal teams and stakeholders to achieve program goals.
  • Analytical: Apply critical thinking to analyze and interpret information and/or data. Ability to analyze qualitative and quantitative information for decision support.
  • Analytical: Demonstrates good decision-making/problem solving skills. Exhibits good clinical reasoning skills and judgments in managing patients with the highest medical complexity/acuity.
  • Communication: Ability to convey and/or receive written/verbal information to/from various audiences in different formats. Communicates effectively and engages in positive inter/intra departmental interaction with peers, physicians, staff and patients.
  • Documentation: Demonstrates the ability to accurately and promptly document all clinical interventions, assessments, and therapy changes in the patient’s Electronic Health Record (EHR).
  • Project Management: Initiate, plan, design, implement, and monitor project(s) Ability to manage projects, establish timeliness, analyze data relevant to areas of responsibility. Ability to function independently, ability to plan, organize, direct, monitor and evaluate assigned work.
  • Budgetary & Financial: Possesses fiscal management skills and proven track record to prepare, monitor and control budgetary expenses and generate revenue.
  • Regulatory: Ability to interpret and apply knowledge of State, Federal and Agency standards to comply with regulations.
  • Technical: Ability to learn and adapt to the use of software, technology, and automated information systems in daily practice.

Physical Demands:

  • Frequent sitting; walking, bending; reaching, occasional pushing, pulling, lifting of materials up to 50 pounds.
  • Responsiveness to auditory alarms and communication devices including telephones, pagers, fire alarms, and mechanical failure alarms.
  • Sufficient mobility to access equipment, patients, and other customers.
  • Able to read information on medication labels.
