Overview

The Social Worker 2 will primarily work on the Community Health Care Van's grant funded program for the treatment and cure of hepatitis C and will provide mental health and wrap-around services. May work on other projects and provide clinical care as needed.

Responsibilities include: Provides patient-centered, trauma-responsive whole-person care for people with substance use disorders. Offers therapeutic counseling modalities. Conducts bio-psychosocial assessments for mental health, substance use, and health related social needs and creates collaborative care plan with clients and patients. Conducts Syndemic Screening Tool. May provide counseling and assessments in person in the clinic or via telehealth. Mobile health visits may be required on occasion. Provides Case Management support for wrap-around services and entitlements (housing, food, insurance, etc.). Provides guidance and oversight for Outreach Workers / Case Managers (including for Ryan White Part A) for case management. Recruits and oversees program Peer(s) Recovery Support. Connects patients with Peers for help with supported recovery and navigation to other community resources. Assists patients in executing care plans to holistically address needs identified in Syndemic Screening Tool. Establishes referral linkages with community resources (in New Haven and other areas in Connecticut - Hartford, Waterbury, New London, and Bridgeport) and provides and tracks referrals. Oversees Community Advisory Board of clients. May oversee educational or support groups. Participates in grant writing and program reporting as needed. May travel as needed to New Haven and other Connecticut outreach locations, case management appointments, and collaborative partner and funder meetings and trainings. Required Certifications: Licensed Clinical Social Work degree with active license to provide therapy.

Required Skills and Abilities

  1. Experience providing therapy in multiple modalities.
  2. Extensive knowledge of medications for the treatment of substance use disorders and serious mental illness. Extensive knowledge of overdose prevention and infectious disease prevention.
  3. Experience with people served by Community Health Care Van: serious mental illness, homeless, substance use disorder, people with HIV and / or hepatitis C.
  4. Experience working on grant funded programs.
  5. Experience and knowledge of clinical research.

Preferred Skills and Abilities

  1. Three + years of experience in a psychiatric or clinical care setting, or equivalent combination of education and experience.
  2. Demonstrated knowledge of Connecticut Community resources.

Principal Responsibilities

  1. Plans and coordinates services for family and group therapy programs, parent groups and discharge programs.
  2. Provides direct psychotherapeutic treatment to severely ill individuals and families and supervises other assigned staff members.
  3. Completes psychosocial assessments, mental status examinations, and diagnostic assessments and translates this information into medically necessary, multidisciplinary treatment plans.
  4. Provides individual and group therapy consistent with evidence-based practice protocols for primary mental health, addiction, and co-occurring populations.
  5. Develops new techniques for family treatment and clinical supervision of students. Anticipates problems and determines priorities.
  6. Evaluates treatment progress and presents findings and recommendations for change at treatment team meetings.
  7. Coordinates and provides clinical case management services, including referrals to appropriate levels of care.
  8. Provides crisis intervention and clinical triage to facilitate necessary services, such as inpatient hospitalization, when indicated.
  9. Researches, develops and implements programs which enhance both inpatient and outpatient services of unit. Monitors discharge status of patients.
  10. Collaborates with families, medical and clinical staff, administrators, insurance agencies, etc. to assure their integration into the treatment planning process, as indicated.
  11. Supervises a staff of exempt and non-exempt employees.
  12. May perform other duties as assigned.

Required Education and Experience

Master's Degree in Social Work and two years of experience in a psychiatric care setting or the equivalent combination of experience and education.

Required License(s) or Certification(s)

State of Connecticut Licensed Clinical Social Worker (LCSW) or State of Connecticut Licensed Professional Counselor (LPC).
