Performs complex diagnostic evaluations upon physician referral and in accordance with departmental and professional guidelines and standards. Verifies appropriateness of referral, contacting provider as necessary. Reviews patient medical history/record and selects, administers and interprets a broad range of standardized and non standardized tests and measurements in order to assess patients' functional status as related to cardiopulmonary, developmental, musculoskeletal, neurological and sensorimotor function. Synthesizes, interprets and analyzes evaluation findings incorporating the patient and family profile.
Demonstrates proficient level of clinical reasoning with development and implementation of individualized treatment programs for patients, including those with complex, rare or multiple dysfunctions. Prioritizes treatment goals and administers appropriate physical therapy procedures and modalities to address goals. Prepares discharge plans and home treatment programs. Provides instruction and education to patients and families. Monitors treatment outcomes, modifying program as needed and communicating recommendations to physicians and other providers. Utilizes advanced clinical reasoning skills for selection/fabrication of orthotics, adaptive/assistive equipment and technology for patients, including those with complex, rare or multiple dysfunctions. Prioritizes patient/family goals in order to meet specific treatment objectives.
Communicates with equipment designers and/or vendors regarding equipment specifications and patient needs. Trains patients, families and/or caretakers in use/operation of prescribed orthotics/equipment, monitors and evaluates effectiveness and initiates modifications as needed. Records assessments, diagnoses and treatment plans in patient medical record according to established documentation standards. Prepares reports, correspondence, referrals and other materials for schools, providers, payors and others as needed. Assigns standardized billing codes and corresponding charges required for reimbursement. Assists in planning and organizing daily clinical operations within designated area. Assists in determining patient assignments and treatment priorities and communicates to patients/caregivers, physicians and other providers.
Performs designated operational duties in absence of Physical Therapist III. Self identifies and develops clinical knowledge and skills through participation in professional organizations, attending continuing education programs, and other related internal and external resources. Integrates professional development into clinical practice and shares knowledge with the department. Attends and contributes to team meetings, patient care rounds and other interdisciplinary forums and provides physical therapy consultation to physicians, specialty clinics and others as requested. Represents service on committees, at meetings and educational programs within the department, hospital and community. May contribute to advancement of profession through participation in research, teaching or advocacy.
Serves as a resource in general pediatrics as well as area(s) of clinical expertise. Participates in departmental training and staff development programs. Serves as a preceptor for new physical therapist staff and as a clinical instructor in student externship program. May act as a mentor and/or faculty member for the clinical residency program. Plans learning objectives, determines work/learning.
