"Case Coordinator, CARE - Part Time"

The Office of Student Conduct is seeking part-time Case Manager for C.A.R.E. The purpose of this class is to provide comprehensive case management support services to the CCBC CARE Team with a focus on individuals in need of intervention for students experiencing academic, personal, or medical difficulties.

Qualifications

Bachelor's degree in student affairs, student development, mental health related field, or other related degree required. Master's degree preferred. Two years experience providing support for students experiencing academic, personal, or medical difficulties or other crisis required. Experience in the higher education and community college environment, preferred. Working knowledge of mental health, student development, Behavioral Intervention Team (BIT) best practices, higher education law, FERPA, and electronic record-keeping systems such as Maxient, preferred. A valid driver's license is required for this position.

Duties

1. Chair and coordinate the CARE team at CCBC.
2. Provide case management services to students who are eligible by the CARE team's recommendation.
3. Coordinate action plan, referrals, and support, including follow-up appointments as necessary.
4. Connect students with the appropriate resources on- and off-campus.
5. Communicate and collaborate with appropriate staff, resources, and the Team.
6. Document case notes in electronic record-keeping system.
7. Demonstrated excellent verbal, written, interpersonal and clinical skills in order to interface effectively with a wide range of students.
8. Maintaining up-to-date case management records in Maxient for students referred to CARE.
9. Point of contact and trainer of the CARE team on best practices and new tools introduced.
10. Interface with faculty, staff, and parents regarding students needing treatment plans, accommodations, or leaves.
11. Create marketing, awareness, and educational campaigns promoting case management services.
12. Develop and deliver presentations tailored to specific stakeholders.
13. Coordinate with institutional offices, ensuring effective integration of messaging.
14. Participate in assessment efforts related to case management effectiveness.
15. Perform other duties as assigned.
