Job Summary

The Intensive Outpatient Clinic (IOC) at University of Utah Health is seeking a Clinical Care Coordinator to tailor and direct critical medical, behavioral health, and social services for IOC patients and their families - ensuring the best possible treatments and outcomes for the population we serve. An integral part of the interdisciplinary IOC team, the Clinical Care Coordinator is a patient advocate and care partner and works across the University of Utah Health and community systems to connect patients to a suite of wrap-around services that improve their overall health, well-being, and flourishing in the community.

Providing comprehensive and integrated care, the IOC operates as an interdisciplinary team of medical and behavioral health providers aiming to improve clinical outcomes and decrease the overall cost of care for the population it serves. In 2023, Governor Cox's One Utah Health Collaborative recognized the IOC as a leader in novel comprehensive care delivery and sustainable financial innovation. The IOC was also featured in Meet Me Where I Am - a short film that premiered in the 2022 Sundance Film Festival - that tells the story of one IOC patient and how this clinic provides unique integrated care.

Working collaboratively with the IOC medical director and interdisciplinary care team, the Clinical Care Coordinator will ensure the successful delivery of exceptional, patient-centered, and integrated care for a targeted population identified as high-risk for poor health outcomes and high utilization of healthcare services. The IOC team is comprised of physicians, advanced practice clinicians, care managers, social workers, and medical assistants. This team works together to deliver high-quality, comprehensive care oriented around treating the bio-psycho-social needs of the IOC patient population.

At minimum, the ideal candidate will have a working knowledge of trauma and substance use disorders, insurance enrollment and claims processes, strong connections with community and social programs, and demonstrated success in coordinating patient care within a multidisciplinary clinical setting. Case management experience is preferred.

The IOC exclusively serves a Medicaid population and operates under a capitated model with the University of Utah Health Plans.

This position reports to the IOC Medical Director and will support the IOC patient population and the clinic team located at the University of Utah Population Health Center near Rose Park.

This position is considered Category I: Mandatory and required on campus or health care facilities to support on-site operations. Employees hired into this category will be required to be physically located at 168 North 1950 West, Salt Lake City, UT 84116 if they are healthy and without COVID-19 symptoms.

Responsibilities

  • Coordinates and connects patients to various resources and support services, such as community organizations, housing assistance, financial aid, etc.
  • Advocates for patient needs & rights, ensuring they receive appropriate care and assistance.
  • Directs the scheduling of patient appointments with providers and ancillary services.
  • Follows up on patient's treatment, ensuring that patient needs are being met and that they understand the course of their treatment.
  • Triages patient questions and ensures appropriate coordination and follow through.
  • Evaluates referrals from physicians and self-referred patients.
  • Gathers needed medical information such as records, slides and films to assist in the evaluation of patient needs.
  • Coordinates treatment-planning conferences and presentations.
  • May develop written protocols for communication to patients regarding treatment plans.
  • May assist with communicating the treatment plan to patients.
  • May act as a liaison with other care groups such as social work, financial counseling and nutrition.

Challenges encountered by this position include creating patient and team resources; complex scheduling with multiple physicians, coordinating treatment-planning conferences for physicians, locating missing medical records and helping patients resolve billing issues. At the delegation of the Chief Population Health Officer, this position is expected to determine how to process the patient through the system in the most effective way. This may include deciding which provider will see the patient and which physician to contact regarding side-affects or health related problems patients may experience.

Working Conditions and Physical Demands

Employee must be able to meet the following requirements with or without an accommodation. This is a sedentary position in an office setting that may exert up to 10 pounds and may lift, carry, push, pull, or otherwise move objects. This position involves sitting most of the time and is not exposed to adverse environmental conditions.

Minimum Qualifications

Bachelor's degree in Health Care Administration, Business, Communication, Nursing, related field or equivalency (one year of education can be substituted for two years of related work experience) required. Demonstrated human relations and effective communication skills are also required. Experience in primary care, multidisciplinary clinics, or working with high-acuity, complex patients may be preferred. This position is patient-sensitive and must fulfill all associated vaccination requirements, or be approved for an exemption, prior to the first day of work. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.
