"Clinical Nurse II -Clinic - Internal Medicine- PT Days"

Incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. Assists with procedures, patient education, outpatient and post hospital discharge follow-up including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation.

Required Qualifications: Previous experience in urgent care, emergency room or critical care setting; Must possess the skill, knowledge and ability essential to the successful performance of assigned duties; Must demonstrate customer service skills appropriate to the job; Minimum two years demonstrated nursing experience in an ambulatory care setting; Minimum of six months demonstrated nursing experience; Demonstrated ability to triage patients and assign level of care; Current California RN license; All external candidates must have a Bachelor of Science in Nursing; Ability to work variable shifts including evenings, nights, weekends, and holidays; Ability to read, write and communicate effectively in English; Ability to maintain flexibility and work well in a constantly changing environment; Ability to maintain a work pace appropriate to the workload; Ability to establish and maintain effective working relationships across the Health System.

Preferred Qualifications: Understanding of applicable regulatory requirements; Pediatric ambulatory experience; Bilingual skills to communicate effectively with patients and families.

Licenses and Certifications: Basic Life Support.
