Salary Range: $106,000.00 - $114,000.00

Position Summary

Responsible for ensuring optimal continuity of care for patients and their families through support, coordination and education of care, fostering collaboration among physicians and the broader interdisciplinary care team participating in clinical activities. This position provides health care including clinical assessment and follow up.

Job Responsibilities

• Responsible for patient coordination. Maintains an overview of patients through the continuum of care. Serves as a team leader in conducting interdisciplinary patient care.
• Responsible for direct patient care. Assesses patient clinical status and triages patients to appropriate physicians or care providers. Troubleshoots patient care issues and communicates with appropriate staff regarding unresolved care issues.
• May coordinate care for patients with acute illnesses and complete appropriate treatments for care episodes. Manages completion of diagnostic testing, ensures timely patient follow-up and communicates actively with referring physicians.
• Ensures timely follow-up strategies. Consults with other members of the nursing staff or clinical care team for patients with complex health problems requiring specialized care.
• Manages Quality metrics in conjunction with Hospital initiatives. Collects and analyzes data from a variety of sources including internal and external benchmarking data, research findings and outcome data to enhance services and improve patient outcomes.
• Maintains a working knowledge of the requirements of payers and services accessible to patients participating in common plans.

Education

• Bachelor's Degree in Nursing

Experience

Approximately 3-5 years of related experience

Knowledge, Skills and Abilities

• Must be articulate and possess a professional demeanor.

Licenses and Certifications

• Current NY State Registered Nurse License & Registration.
• BLS Certification (issued by the American Heart Association)
• Infection Control Certificate.
