LICENSED PRACTICAL NURSE (Ambulatory Care: Centralized Medication Refill )

- Answer incoming calls and electronic messages related to medication refills.
- Verify patient identity and confirm prescription details.
- Document all refill requests and communications accurately in the electronic medical record (EMR/EHR).
- Utilize electronic protocol database to review and process routine refill requests following clinical protocols or standing orders
- Collaborate with providers when necessary and route requests requiring provider approval or consultation to appropriate clinical staff.
- Communicate prescription status to patients and pharmacies.
- Collaborate with providers, nursing staff, and pharmacy personnel to resolve discrepancies and support patient needs.
- Maintain knowledge of insurance regulations, medication formularies, and refill guidelines.
- Adhere to HIPAA and all privacy, confidentiality, and patient safety regulations.
- Provide professional, compassionate, and informative customer service to patients and families.
- Adapt communication style to meet the needs of patients from diverse personal, professional, cultural, and socioeconomic backgrounds.
- Use excellent listening skills to receive and interpret detailed information from patients
- Program Responsiveness: Support program needs, including but not limited to, mentoring/orienting new staff, adapting to shifts in technology, workflow, and organizational priorities as the unit evolves and grows.
- Meet or exceed established quality and productivity standards.
- Demonstrate knowledge of and appropriately use internal and external resources.
- Provide functional supervision to assistive personnel and collaborate with colleagues, other disciplines, and community resources to ensure continuity of care.
- Perform other related duties as assigned.
- All meetings related to this position will be conducted in person or, when remote participation is required, staff will be expected to be on camera and actively engaged.

- Proficiency with typing and electronic health record documentation.
- Willingness to adapt to different populations i.e.: pediatrics, adult medicine as patient demands. Orientation and training would be provided for new subspecialties within primary care based on expansion of division.
Required Qualifications
- Graduated from an approved school for practical nursing and a current licensure a Licensed Practical Nurse (LPN) in the State of Michigan is necessary.
- Minimum of one (1) year of nursing experience in the past five (5) years in healthcare, pharmacy, or call center setting.
Preferred Qualifications
- Familiarity with EMR systems. (EPIC).
- Minimum of one year of ambulatory care experience.
- Experience working with UMHS Policies.
- LPN experience within an ambulatory care setting (within the past five (5) years).
- Bilingual
Hours/Week: 40 Hours
Shift/Hours/Days: Day Shift; includes weekends and holidays.
Location: North Campus Administrative Complex (NCAC)
This program is primarily considered an on-site position, there may be opportunities to work in a hybrid capacity depending on organizational policies and the evolving needs of the Centralized Medication Refill Line.
