"Nurse Practitioner - Cardiology"

Position Summary

The Department of Medicine is seeking a Nurse Practitioner (NP) to work within the outpatient practice of the Cardiac Amyloidosis Program in the Center for Advance Cardiac Care (CACC) under the management of the Director of the Cardiac Amyloidosis Center within the Division of Cardiology. The NP will work collaboratively with Physicians, NP and PA peers, Registered Nurses, other members of multidisciplinary care team, and administrative support staff. The NP will render direct patient care during outpatient office hours including but not limited to obtaining patients' history, performing comprehensive and targeted physical examinations, ordering and evaluating diagnostic tests, and diagnosing/managing chronic as well as acute patient problems. The NP will also be responsible for counseling/educating patients and their caregivers on disease processes and care management as needed. The provider will be engaged in all aspects of patients' clinical care and work with other members of the care team to provide seamless care across all venues including outpatient, inpatient and clinical research venues.

Responsibilities

• Collaboratively and autonomously facilitate and deliver care to a specific subset of patients with all forms of Cardiac Amyloidosis as well as other cardiomyopathies.
• Obtain and review outside records/reports as needed and follow-up/collaborate with the Physicians to re-develop the treatment plan as necessary.
• Work with NP and PA peers and the Registered Nurse team to triage daily patient calls and communicate with referring Providers.
• Work with the multidisciplinary team including but not limited to Social Workers, Nutritionists, Pharmacists, Surgeons, the Palliative Care Team, the CACC inpatient team, clinical research team and other specialists as necessary to provide/coordinate care across the continuum, including in-patient admissions and clinical research protocol visits.
• Document all patient/caregiver encounters in the appropriate record.
• Participate in weekly Team meetings.
• Perform other responsibilities as assigned.

Minimum Qualifications

• Requires a master's degree in nursing as a Nurse Practitioner trained in the care of adults, plus one to two years of experience in a hospital setting, preferably In Cardiology.
• Current New York State (NYS) NP license (or proof of application) and RN license.
• Must have NP Board certification.
• Current NPI certification.
• Must be eligible for DEA certification.
• Must be eligible for ANCC Board Certification.
• Must be eligible for AHA ACLS & BCLS.

Preferred Qualifications

• Adult Gerontological Care experience preferred. Experience with the Cardiology patient populations preferred.
• Proficiency in spoken and written Spanish preferred.

• Knowledge of a variety of computer and software applications [Microsoft Office, REDCap, electronic data entry systems i.e. Medidata]
• Ability to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.
• Ability to handle confidential information and maintain confidentiality in accordance to HIPAA.
• Participation in Medical Surveillance Program + Contact with patients and/or human research subjects. + Wear N-95 respirator + Potential bloodborne pathogen exposure
• Successful completion of applicable compliance and systems training requirements.
