Nursing Care Coordinator
Rutgers, the State University of New Jersey, is seeking a Nursing Care Coordinator for the Medicine Department within New Jersey Medical School. The primary purpose of the Nursing Care Coordinator position is to coordinate and deliver specialized nursing care services. Working within a healthcare setting, you will be responsible for ensuring that patients receive timely, compassionate, and evidence-based care, while also addressing their unique physical, emotional, and social needs. This role involves collaboration with a multidisciplinary team to optimize patient outcomes and enhance the overall quality of care.
- Conduct thorough assessments of patients living with and/or at risk for HIV/AIDS to determine their healthcare needs.
- Facilitate communication between healthcare providers, specialists, and support services to optimize patient care.
- Provide education and counseling to patients and their families regarding HIV prevention, treatment options, and adherence to care plans.
- Implement and promote strategies for HIV transmission prevention and risk reduction.
- Communicate regularly with patients to monitor progress, address concerns, and reinforce health management strategies.
- Coordinate healthcare services, referrals, and follow-up care to ensure continuity of care.
