PS Case Analyst
Job Summary
Coordinates services for patients of the Free PrEP Clinic as directed by attending or referring physician or clinical case manager. Oversees delivery of individualized services to patients. Manages and reports on aggregate patient data and represents the clinic in collaborations with stakeholders in the community.
Responsibilities
- Functions as the patients' advocate to facilitate effective communication and appropriate treatment.
- Assesses patients for eligibility for Free PrEP Services
- Orients patients to the Free PrEP Clinic.
- Works with health care team, including the patient, to assist the patient in gaining access to PrEP Medications.
- Advises and assists patients in accessing financial and community resources.
- Assists in establishing and maintaining good relationships with community stakeholders, physicians, third party payers and other community agencies.
- Maintains systems for aggregation of clinic population data, completion of evaluations and reports and scheduling of team meetings.
Problem Solving
This position is responsible for facilitating open and accurate communication among patients, referral providers, payers, and the health care team.
Minimum Qualifications
Bachelor's degree in a clinical area, health care administration, or related field, or equivalency (one year of education can be substituted for two years of related work experience); two years of experience in case management activities with third party payers; and demonstrated organizational, human relation and effective communication skills required. Licensure to practice in a clinical field preferred.
Preferences
Fluency in Spanish is preferred. This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations. The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
