Key Responsibilities:

  • Conduct thorough review of medical records to determine if potential candidates meet the specified criteria for kidney and/or pancreas transplants.
  • Utilize your communication skills to connect with newly referred patients over the phone, assessing their medical history and candidacy for transplantation.
  • Collaborate with a hard-working, multi-disciplinary team, providing valuable insights and support during the referral phase, while ensuring all vital information is meticulously gathered and detailed.

Qualifications

Education:

Graduate of an accredited nursing program required
BSN required

License/Certifications:

Current, unrestricted California RN License required
BLS from the American Heart Association required
Specialty or Transplant certification required or willing to acquire within one year from start date

Experience:

A minimum of 3 years of Transplant or Acute Care experience required
Proven knowledge of case management required
Experience in data management required
ICU experience preferred
