Organizational Unit Overview:
The Division of Infectious Diseases at The Brody School of Medicine provides state-of-the-art diagnosis and treatment for a wide range of infections. Infectious Disease doctors help ensure the timely diagnosis and treatment of acute infections by recommending the appropriate diagnostic tests to identify the source of infections and by recommending the appropriate antibiotic to treat the infection. We are dedicated to the treatment of persons infected with HIV in Eastern North Carolina. Our physicians and physician extenders currently provide care for more than 1,500 patients in our clinic in Greenville as well as another 300 patients in clinics in surrounding counties. Faculty members are currently collecting data to address the issue of the disproportionate infection rate of HIV in African Americans. The division is involved in many clinical trials involving HIV therapies. These trials provide needed drugs to persons who may otherwise not be able to receive treatment.
Job Duties:
The Case Manager - Patient Navigator is part of the Brody School of Medicine HIV Program (BSOMHIVP) to link and retain people living with HIV/AIDS (PLWHA) in care. The individual will provide a range of client center services to retain PLWHA who are in care at BSOMHIVP by supporting PLWHA who are newly diagnosed and those who have had a lapse in health care. Will also support clients with assessing and navigating the new federal health care insurance exchange system in North Carolina. This is a grant-funded position within the Division of Infectious Diseases at East Carolina University, Department of Medicine. The Medical Case Manager is primarily a self-directed position and will work in collaboration with other members of the ECUHIVP treatment adherence team to link and retain PLWHA in HIV care. Will work under the direct supervision of the ECUHIVP Ryan White program administrator. Work activities are both individual and community based. The individual will assess clients for barriers to HIV care and developing an action plan to address barriers identified; monitoring care plan objectives, and assist clients with accessing medical services, mental health, and substance abuse services; educating clients on HIV disease and how to become actively engaged in their care; identify clients that have fallen out of care, tracking these clients and encouraging re-engagement in care; and coordinate support services that will help clients to retain in care. Will work with clients to complete the Ryan White eligibility process and track clients that do not show for their provider appointments to reschedule provider appointments. Will assist clients to find affordable HIV medications by enrolling clients in the HIV medication assistant program, 340b medication program, and drug company prescription assistance programs. Due to the nature of the job, the work schedule is flexible, including possible nights, and weekends.
CLINICAL SUPPORT POSITION - NOT SUBJECT TO STATE HUMAN RESOURCES ACT.
Employment in this position provides eligibility to participate in the UNC Optional Retirement Program (ORP) which offers matching employer contributions and employee control of investment choices, along with eligibility to participate in various Voluntary Supplemental Retirement Plans (401k, 403b, 457). This position is not eligible to participate in the NC Teachers and State Employees Retirement System (TSERS). Additionally, this position offers multiple voluntary options for Medical, Dental, Vision, and Life Insurance as well as various supplemental plans including Critical Illness, Flexible Spending Accounts, Cancer, and Disability.
While this position is not automatically eligible for the NC Teachers and State Employees Retirement System (TSERS), new hires transferring from another North Carolina state agency and current ECU employees transferring internally who had been enrolled in TSERS in their previous employment will continue participation in TSERS. These individuals may be able to elect to participate in ORP if they had not been offered that option at the time of their initial enrollment into TSERS.
Minimum Education/Experience:
Associate degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
License or Certification Required by Statute or Regulation:
None
Preferred Experience, Skills, Training/Education:
Experience in case management working with people with HIV disease.
Knowledge of HIV/AIDS
Experience in public speaking and interpersonal communication; bilingual applicants, particularly Spanish speaking, is an asset.
Working knowledge of electronic medical databases such as Health Span, Centricity and CAREWARE is desirable
