Licensed Vocational Nurse - Neurology
The Licensed Vocational Nurse (LVN) in Neurology plays a critical role in supporting patients and providers. Working under the direction of the RN Navigator and Nurse Manager, the LVN provides clinical support, patient education, and coordination of care. This role involves rooming patients, triaging provider messages, preparing patients for procedures, and ensuring continuity of care within the practice.
Key Responsibilities:
Patient Care & Clinical Support
- Assess and identify patient needs through observation and communication with the care team.
- Assist with rooming, intake, and patient education on diagnostic and therapeutic procedures.
- Administer and document medications safely; maintain knowledge of pharmacology and modes of administration.
- Respond appropriately in emergencies, participate in safety drills, and maintain emergency equipment readiness.
- Set up and maintain exam rooms, ensuring cleanliness and proper supply levels.
Care Coordination & Communication
- Support review of new patient referrals and triage provider in-basket messages.
- Maintain effective patient flow and coordinate scheduling to ensure timely care.
- Facilitate continuity of care through clear documentation, patient follow-up, and communication with providers and families.
- Secure prior authorizations for medications, visits, infusions, and procedures.
- Participate in daily huddles and support practice quality initiatives, including Patient-Centered Medical Home recognition.
Patient Education & Advocacy
- Provide patients and families with information and resources to support informed decisions.
- Utilize language services and culturally sensitive communication when appropriate.
- Reinforce health teaching and verify patient/family understanding.
- Participate in patient action planning and health coaching activities.
Professionalism & Performance Improvement
- Uphold UCSF PRIDE values and AIDET communication standards in all interactions.
- Participate in performance improvement activities and recommend process enhancements.
- Attend staff meetings, trainings, and continuing education opportunities.
- Support orientation and mentoring of new staff.
