We are seeking a compassionate, dedicated Inpatient Transplant Coordinator to complete the following job duties with our inpatient liver transplant population.

Essential Duties:

• Responsible for the planning and coordination of all aspects of liver transplant patients’ care from referral to post transplant.
• Serves as the primary coordinator for the interdisciplinary team, including physicians, nurses, social workers, dieticians, pharmacists, financial analysts, case managers, and support staff.
• Develops, coordinates, implements, and evaluates educational programs/projects for patients, personnel, and the community.
• Collects and documents data to meet clinical, financial, and regulatory requirements.
• Participates in the development of guidelines, protocols, pathways, and other performance improvement activities.
• Participates in research activities.
• Serves as a patient advocate throughout the transplant/surgical process.
• Represents the Medical Center at regional and national meetings/conferences.

Qualifications

Education:

• Graduate of an accredited nursing program required.
• BSN required.

License/Certifications:

• Current, unrestricted California RN License required.
• BLS from the American Heart Association or American Red Cross required.
• Specialty or Transplant certification required within one year from start date.

Experience:

• A minimum of 3 years Acute Care or Critical Care experience required.
• Experience as a Transplant Coordinator preferred.
