Job Title:

Community Outreach Coordinator II

Job Purpose:

Plans, coordinates, and delivers in person and remote functional behavioral assessments to support the state implementation of the Home and Community Based Supports (HCBS) Waiver system. This includes travelling throughout the state to collect the Inventory for Client and Agency Planning (ICAP), or similar functional assessments, in client homes or other community settings.

Essential Duties and Responsibilities:

  • Conduct in-person assessments of individuals who are eligible for, or who are receiving services through the Home and Community Based Waiver system.
  • Use scoring software to compute scores, write reports and deliver feedback to the state of Wyoming using a web portal.
  • Develop and maintain cooperative working relationships with clients, state agencies, case managers, direct support professionals, family members, and other individuals who serve those with intellectual and/or developmental disabilities.
  • Drive a fleet vehicle throughout Wyoming.
  • Monitoring supply inventories.

Remote Work Eligibility:

This position can be based in any part of Wyoming, but the candidate must be able to travel to conduct ICAPs, as well as make occasional trips to Laramie, WY for departmental activities.

Minimum Qualifications:

  • Education: Bachelor's degree in disabilities studies, human/child development, psychology, social work, public health or other fields related to disabilities or human services.
  • Experience: 2 years progressively responsible work-related experience. Experience must include at least 2 years at the preceding level or equivalent. Experience with clinical interviewing, standardized assessments, or other objective measures of human development and/or behavior.
  • Required licensure, certification, registration, or other requirements: Valid driver's license with no major infractions or other limitations that could limit driving a fleet vehicle.

Desired Qualifications:

Knowledge of:

  • Aging, healthcare, social services, and/or chronic disease prevention and management programs.
  • Assessment and data analysis techniques, methods, and practices.
  • Departmental computer technologies, including system requirements, functions, upgrades, additions, and new hardware and software developments.
  • Assessment techniques, interviewing skills and objective behavioral evaluation.

Skills and Abilities to:

  • Understand, interpret, and implement Federal, State and University policies and procedures.
  • Apply problem-solving techniques and professional judgement in complex, confidential and sensitive situations.
  • Access, establish and maintain a network of community agencies for resources and referral purposes.
  • Work effectively with faculty, staff, students, families, and other customers from different backgrounds.
  • Adhere to high standards of integrity, confidentiality, and honesty.
  • Work as a team member and foster a cooperative team environment.
  • Ability to objectively assess functional behaviors and provide justification for assessments using observed information from the interview.
